- Place name information on top line
. Voters instinctively want to
fill in their name first, and often skip the top line spaces for birth
date and social security number.
- Brief explanatory note after suffix
. Many voters do not understand
that this refers to "Jr." or "Sr." and will write
"Mr." or "Ms."
- Eliminate space for county
. This can be determined based on
address. Many voters read this as "country" and write
"USA" in the blank.
- Place date of birth on the left end of the row below name
. This is
the most commonly forgotten or wrong piece of information. The current
placement after social security number seems to encourage voters to skip
the space.
- Brief explanatory note on mailing address line
. Many voters,
especially students, misinterpret this line. Often they will list what
appears to be their parent’s address, which can lead to confusion as to
where the voter intends to register. It should be more clear that this
line is for post office boxes or mail drop locations.
- Eliminate space for school district
. This can be determined based
on address. Many voters assume this space means a specific school
building, or simply have no idea what school district they live in.
("University of Iowa" is also a common response.)
- Move Signature line to bottom of form
. Voters often miss the
request for previous registration information.
- Glue Spot Placement
. Occasionally voter information adjacent to
the glue spots is torn when the form is opened.
