1) COMPLETE the Application form
2) Application must be NOTARIZED
3) Include copy of valid PHOTO ID
4) Certified copy cost: $15

Marriage Record Application Form

Marriage records begin at the year 1839.  Certified copies are $15 and only available to entitled parties (i.e. person named on record, parents, children, spouses, siblings, grandparents).  Individuals requesting a marriage record must prove entitlement by showing proper identification.

Marriage records between the years 1921 – 1941 must be requested through the Iowa Department of Public Health.
